Microbiological profile and bacterial resistance in a public hospital: challenges for antimicrobial therapy

Rising bacterial resistance to antimicrobials poses a critical challenge for the control of hospital-acquired infections. This retrospective, observational study analyzed 2,250 microbiological culture and antibiogram results from a public hospital in southwestern Bahia, Brazil, between 2015 and 2016. Samples of blood, catheter tips, urine, and tracheal aspirates were evaluated, providing a comprehensive overview of the institution's microbiological profile. Coagulase-negative staphylococci were the most prevalent microorganisms. Among Gram-positive bacteria, *Staphylococcus aureus* stood out due to its high rate of multidrug resistance, driven by the presence of methicillin-resistant *S. aureus* (MRSA). In the Gram-negative group, *Pseudomonas aeruginosa* demonstrated carbapenem resistance via metallo-beta-lactamase (MBL) production, while *Klebsiella pneumoniae* exhibited the highest percentage of multidrug resistance among all analyzed strains. Notably, Polymyxin B was the only antibiotic evaluated against which no resistance was observed. These findings underscore the urgent need for effective microbiological surveillance strategies and the rational use of antimicrobials to curb the spread of bacterial resistance and optimize empirical therapy for hospital-acquired infections.
