The objective was to estimate the prevalence and investigate the factors associated with smoking among pregnant women in Santo Antônio de Jesus, Bahia. This is a cross-sectional study nested in a cohort of pregnant women in the municipality of Santo Antônio de Jesus, Bahia, with a sample of 1,156 women seen at sixteen Family Health Units between 2012 and 2015. Information on smoking and socioeconomic, demographic, reproductive health and lifestyle characteristics was obtained using a standardized questionnaire. Smokers were those who reported smoking during pregnancy, regardless of the frequency and duration of the habit. The data was entered into EpiData version 3.0 and analyzed using STATA version 12.0. The association between smoking and maternal characteristics was analyzed using the logistic regression technique. The magnitude of the association was estimated by calculating the crude and adjusted odds ratios (OR) with a 95% confidence interval (95%CI). The prevalence of smoking during pregnancy was 3.3% (95%CI: 2.0-4.0). The factors associated with smoking were: being multiparous (OR=7.1; 95%CI: 2.6-19.9), alcohol use (OR=5.1; 95%CI: 1.2-22.1) and use of other drugs (OR=8.6; 95%CI: 2.4-30.6). Compared to other studies, the prevalence of maternal smoking was considered low. However, more emphatic interventions need to be targeted at pregnant women with the vulnerability factors found.
